Applicants must:

• Have reliable transportation and be willing to travel throughout all 15 NWGA counties.
• Be comfortable visiting consumers' homes, nursing facilities, schools, businesses, and community events.
• Be proficient with Microsoft Office 365, email, online databases, and electronic documentation.
• Be capable of managing a full caseload independently.
• Successfully pass a criminal background check.

Position Summary

NWGA Center for Independent Living is seeking a professional who is dependable, self-directed, and committed to serving individuals with disabilities. This position requires professionalism, accountability, and the ability to manage a demanding workload while representing the organization throughout the community.

Primary Responsibilities

·         Assist individuals transitioning from nursing facilities into the community.

·         Coordinate housing, equipment, benefits, and community resources.

·         Work closely with hospitals, nursing facilities, waiver providers, and community partners.

·         Maintain accurate WellSky and agency documentation.

·         Advocate for consumers throughout the transition process.

·         Travel extensively throughout Northwest Georgia.

Minimum Qualifications

·         Bachelor's degree or relevant human services experience.

·         Knowledge of community resources and durable medical equipment preferred.

·         Reliable transportation and valid driver's license.

·         Strong documentation and computer skills.

·         Background check required.

·         Represent NWGA CIL professionally in the community.
