This position is not a work-from-home or desk-only position.
Applicants must:
- Have reliable transportation and be willing to travel throughout all 15 NWGA counties.
- Be comfortable visiting consumers' homes, nursing facilities, schools, businesses, and community events.
- Be proficient with Microsoft Office 365, email, online databases, and electronic documentation.
- Be capable of managing a full caseload independently.
- Successfully pass a criminal background check.

Position Summary
NWGA Center for Independent Living is seeking a professional who is dependable, self-directed, and committed to serving individuals with disabilities. This position requires professionalism, accountability, and the ability to manage a demanding workload while representing the organization throughout the community.
Primary Responsibilities
- Assist individuals transitioning from nursing facilities into the community.
- Coordinate housing, equipment, benefits, and community resources.
- Work closely with hospitals, nursing facilities, waiver providers, and community partners.
- Maintain accurate WellSky and agency documentation.
- Advocate for consumers throughout the transition process.
- Travel extensively throughout Northwest Georgia.
Minimum Qualifications
- Bachelor's degree or relevant human services experience.
- Knowledge of community resources and durable medical equipment preferred.
- Reliable transportation and valid driver's license.
- Strong documentation and computer skills.
- Background check required.

- Meet deadlines and complete documentation accurately.
- Communicate effectively with supervisors, coworkers, and community partners.
- Represent NWGA CIL professionally in the community.
- Failure to meet performance expectations may result in disciplinary action.
