Job Title: Case Manager
Position type: Full-time, Non-Exempt
Reports to: Program Manager
Position Summary:
The Case Manager is responsible for managing the needs of women receiving services through Hospitality House for Women in the shelter. Their primary function is to serve as an advocate to our clients and to assist them with identifying needs and goals, as well as means to achieve them. Through a trauma-informed lens, the Case Manager meets each client where they are and partners with them to build a plan for living free from intimate partner violence.
Duties and Responsibilities:
ï‚· Answer a 24-hour crisis line.
ï‚· Case plan with individual clients weekly. Case planning may include goal setting, safety planning,
budgeting, etc.
ï‚· Conduct domestic violence risk and danger assessments to determine appropriate referrals and share
education about resources in the community.
ï‚· Facilitate crisis coordination of services.
ï‚· Assist clients experiencing DV with developing safety plans.
ï‚· Development of trauma-informed case plans.
ï‚· Be open-minded and accepting of individual, cultural, and lifestyle differences.
ï‚· Advocate for the needs survivors.
ï‚· Identify situations in which mandated reporting is necessary.
ï‚· Be self-directed and flexible.
ï‚· Positively receive feedback and utilize it to learn and grow.
ï‚· Self-reflect and identify own strengths, abilities, areas for growth, emotional responses, and goals.
ï‚· Maintain current CPR and First Aid certification.
ï‚· Facilitate peer support groups.
ï‚· Transport clients to appointments in shelter van.
ï‚· Any other duties assigned by Program Manager or Executive Director.
Administrative Duties
ï‚· Complete all paperwork and documentation of services within 48 hours.
ï‚· Adhere to agency policies and work rules including strict adherence to confidentiality policies and code of ethics.
ï‚· Attend and assist agency meetings as required by Program Manager or Executive Director
ï‚· Complete all initial training and continued annual training of at least 20 hours per year.
Qualifications
ï‚· Bachelor’s degree in Human Services or Social Work preferred, or 5 years equivalent experience in social services field.
ï‚· 2-5 Years’ experience working with diverse individuals experiencing domestic violence.
ï‚· Valid driver’s license.
ï‚· Complete and pass all required background checks.
ï‚· Preferred skills include computer, writing and documentation skills, the ability to work as a team, remains calm in times of crisis, problem-solving, positive and effective communication, time management, and a sense of personal and community responsibility.
ï‚· Physical requirements: May require some heavy lifting and otherwise typical office tasks.
