Area Agency on Aging MFP/NHT Transition Coordinator

The Northwest Georgia Regional Commission Area Agency on Aging is seeking a qualified candidate for Transition Coordinator. 

This position is responsible for meeting with individuals who are eligible for the Money Follows the Person Program (MFP) and Nursing Home Transitions (NHT) program that helps a client successfully transition from an institution back into the community.  

Requirements are a Bachelor's degree, preferably in Social Work, or Registered Nurse with a minimum of three years in a Human Services position. 

Preference will be given to those with experience in Nursing Home Transitions, Care Transitions, Information and Referral Assistance, Options Counseling, or related fields.
