Coordinator, Transitional Living
Scope of Position: The Coordinator, Transitional Living manages the cases and goals of each graduate of WinShape Homes and serves as their point of contact assisting and providing accountability for the benefits received towards their pursuit of a college degree, trade, or technical skill/certification. The Coordinator, Transitional Living is also responsible for the documenting and maintaining the student’s case file and providing resources and coaching to support students in the progress strategy.
Reports to: Supervisor, Human Services Professional – Homes
Responsibilities:
- Responsible for having comprehensive knowledge of Transitional Living Guidelines and serves as the main point of contact with students as to implementation requirements.
- Implements the financial procedures for Transitional Living students and ensures on-going compliance with program requirements to maintain financial benefits for each student.
- Develop a progress strategy with each student to provide intentional care and support goals outlined in the progress strategy for each student in the program and meets with each student at a minimum of every other month to discuss progress towards goals, provide coaching, and recommend additional services as needed.
- Provides resources and coaching for independent living skills for needs identified through the progress strategy.
- Oversees additional benefits for students including housing, health insurance, car insurance, cell phone allowance, incidentals, medical expenses and submits requests for emergency assistance with medical needs and car needs for approval by Director of Group Care.
- Initiates process for students who desire to request reconsideration of benefits and presents information to supervisor and serves as liaison for information gathering in the decision making process.
- Ensures tuition is paid on time to each institution and that a log is maintained to track expenditures made each semester, assists with purchases and/or reimbursements for school related purchases such as books and materials.
- Meets with HS Seniors in Group Care program to introduce benefits and assists with transition plans for those pursuing academic programs and assist as needed with life skills assessments and skill training with the GC program.
- Assist as needed with the ILP Residential Community to serve as a member of the team and assist as needed with program and provide consistency in approach between Transitional Living and ILP Students.
- Conducts self in a professional manner with knowledge in multiple aspects of child welfare and transitional living and obtains appropriate training and education to strengthen skillset. Assists team in the execution of staff trainings and retreats. Participates in HSP meetings, WinShape Homes trainings, ONE Conference, Fall Rally, and other WinShape Foundation events.
- Represents and promotes WinShape Foundation and Homes at all times in day-to-day activities and exemplifies our core values.
- Performs other duties and tasks as assigned by Supervisor and Director of Group Care.
